RESUMEN

OBJECTIVE@#Ganoderma lucidum spore (GLS) is gaining recognition as a medicinal part of G. lucidum and has been reported to possess various pharmacological properties, such as antitumor activity. In this work, wall-broken GLS powder (BGLSP) and wall-removed GLS powder (RGLSP), two kinds of GLS powder with different manufacturing techniques, were compared in terms of contents of active constituents and in vivo and in vitro antitumor effects.@*METHODS@#The ultraviolet and visible spectrophotometry method was used to determine the contents of polysaccharides and total triterpenoids in BGLSP and RGLSP. Seventeen individual triterpenoids were further quantified using ultra-high-performance liquid chromatography and quantitative analysis of multi-components by single marker. The antitumor effects of BGLSP and RGLSP were evaluated using in vitro cell viability assay against human gastric carcinoma SGC-7901, lung carcinoma A549 and lymphoma Ramos and further validated by in vivo zebrafish xenograft models with transplanted SGC-7901, A549 and Ramos.@*RESULTS@#The results showed that the contents of polysaccharides, total triterpenoids and individual triterpenoids of RGLSP were significantly higher than those of BGLSP. Although both BGLSP and RGLSP inhibited the three tumor cell lines in vitro in a dose-dependent manner, the inhibitory effects of RGLSP were much better than those of BGLSP. In the in vivo zebrafish assay, RGLSP exhibited more potent inhibitory activities against tumors transplanted into the zebrafish compared with BGLSP, and the inhibition rates of RGLSP reached approximately 78%, 31% and 83% on SGC-7901, A549 and Ramos, respectively.@*CONCLUSION@#The results indicated that the antitumor effects of GLS were positively correlated with the contents of the polysaccharides and triterpenoids and demonstrated that the wall-removing manufacturing technique could significantly improve the levels of active constituents, and thereby enhance the antitumor activity.

RESUMEN

Objective To analyze the problems and reasons in the clinical medicine postgraduate cultivation.We stated the competency of clinical medicine postgraduates according to the transition of health demands global medical development and practical requirements for clinical personnel. Me thods Questionnaire surveys combined with interviews were conducted, and statistical analysis was performed. In the first part, we selected the permanent staff with qualifications of medical clinicians, including: chief physicians, associate chief physicians, physicians, residents, accounting for 72% of the interviewees; And related personnel, including nurses, administrators and patients, were also interviewed, accounting for 28% of the total. 90 questionnaires were distributed, and 86 valid questionnaires were recovered, with an effective rate of 95.56%. In the second part, graduate students with clinical professional degrees were selected for interviews, 96 questionnaires were distributed, and 92 valid questionnaires were collected, and the effective rate was 95.83%.Re s ults There was a certain gap between the expected goal and the actual situation of the post competency of clinical professional degree graduate students, and the gap had a certain statistical significance. The clinical professional degree graduate students paid more attention to the doctor-patient communication, teamwork. Conclus ions We elaborated the required post competence for clinical medicine postgraduates on the basis of its content and characteristic. We propose to strengthen the cultivation of clinical medicine postgraduates by the reform and support of the government enhanced construction of teachers improved course plan and cultivation process perfected assessment system in order to make the students competent to their posts.

AIM To investigate the efficacy and safety of Chinese Jing Liqueur (Curculiginis Rhizoma,Angelicae sinensis Radix,Cistanches Herba,etc.) in relieving main symptoms of patients with Kidney Yang Deficiency Syndrome.METHODS Within eight-week trial,one hundred and twenty patients randomly and equally assigned to control group and experimental group took 50 mL Chinese Jing Liqueur and 50 mL 10% diluted Chinese Jing Liqueur,respectively.The grading scale for Kidney Yang Deficiency Syndrome and change curve for body surface temperature after drinking were established on day 0,the 4th week day and the 8th week day.The blood samples were collected for blood hemorheologies detection as well.An array of measurements before and after drinking,the Kidney Yang Deficiency Syndrome scores,single symptom scores,body surface temperature and hemorheologies between the two groups were thus compared.RESULTS The experimental group displayed a significantly higher clinical cure rate (31.034%) than the control group (5.172%) (P =0.000),total effective rate (69.966%)than the control group (34.483%) (P =0.000).The experimental group didn't compromise its superiority to the control group if evaluated by improvement in chilly sensation and the cold limbs (35.593% to 6.667%,P =0.000);and by the fibrinogen level [(2.845 ± 0.724) g/L to (2.500 ± 0.395) g/L,P =0.004)].No significant difference in incidence of adverse reactions between the two groups was observed (P =0.619).Meanwhile,Chinese Jing Liqueur's power in improving the patients' fatigue and weakness of waist and knees,hyposexuality,listlessness,nocturia and lower extremity edema was noticed as well.CONCLUSION For patients with Kidney Yang Deficiency Syndrome,Chinese Jing Liqueur proves its efficacy in improving their main symptoms through enhancing the basic skin temperature and prolonging the duration of skin temperature rise.

RESUMEN

A chemical investigation on the aerial parts of Hypericum perforatum resulted in the isolation of a new phloroglucinol derivatives (1), and seven known compounds (2-8). The structures of the compounds were elucidated by means of spectroscopic methods (MS, IR, 1D NMR, and 2D NMR) as 3-methyl-4,6-di (3- methyl-2-butenyl)-3-(4-methyl-3-pentenyl)-2-(2-ethyl-1-oxobutyl)-cyclohexanone (1),hyperforin (2),(2R,3R,4S,6)-3-methyl-4,6-di(3-methyl-2-butenyl)-2-(2-methyl-1-oxo-propyl)-3-(4-methyl-3-pentenyl)-cyclohexanone (3),hyperscabrin B (4),hyperscabrin C (5),furohyperforin isomer 1 (6),furoadhyperforin (7),and furohyperforin (8). Compound 1 was a new compound, and compounds 3-5 were obtained from H. perforatum for the first time.

RESUMEN

Objective To establish a suitable model for therapeutic treatment on rats of anti-glomerular basement membrane nephritis. Methods Rabbit with rat glomerular basement membrane and freund’s adjuvant were immuned by multi-point subcutaneous injection biweekly, five times totally, and the antiserum was prepared. Experimental model for therapeutic treatment was established. Serum level of immune complex (CIC), intensity of immunofluorescence of glomerular basement membrane, lesion of kidney and glomerular basement membrane was measured by multiple intravenous injection of antiserum to rats to determine suitable experiment conditions including titer of antiserum, dose of antiserum, method and time of antiserum injection. The model was observed the efficacy using Gushen 2 as therapeutic drug. Results Rats with 4 mL antiserum (titer 1:4) by intravenous injection were immuned 4 times 3 d. Rat anti-glomerular basement membrane nephritis form on day 6, 8, and 10 since immunization; Pathogenic condition were relatively mild on day 6 since immunization and basically stable on day 8 and 10 since immunization. Verification tests showed that, Gushen 2 significantly reduced the 24 h urine protein excretion, relieved kidney lesion for 30 consecutive days, which had obvious therapeutic effect on rats with anti-glomerular basement membrane of nephritis. Conclusion Rats immuned with 4 mL antiserum (titer 1:4) by intravenous, immune points up to 4 times 3 d, rat anti-glomerular basement membrane nephritis model form which are suitable for therapeutic treatment of drug on day 8 since immunization.

RESUMEN

Objective To observe the effects of mind-regulating brain-developing acupoint magnetic therapy in patients with mild cogni-tive impairment (MCI). Methods From May to October, 2015, 60 patients with MCI were randomly divided into control group (n=30) and observation group (n=30). The control group received magnetic therapy on Yangming meridians, and the observation group received magnet-ic therapy on mind-regulating brain-developing acupoints, for two months. They were assessed with Montreal Cognitive Assessment (Mo-CA), digit-span and digit symbol of Wechsler Adult Intelligence Scale (WAIS). Results After treatment, the scores of MoCA, digit-span and digit symbol of WAIS significantly increased in the observation group (t>4.858, P2.188, P5.527, P2.324, P2.262, P0.05). Conclusion Magnetic therapy on acupoints related with cognition could improve the cognitive function in patients with MCI.

Objective To compare the effect of diversification rehabilitation team mode on function in patients with cerebral infarction. Methods Sixty-six patients with cerebral infarction were divided into diversification rehabilitation team mode group (diversification group) and routine rehabilitation mode (routine group) according to the rehabililation method with 33 cases each. All patients of 2 groups were treated for 2 weeks. Evaluations were made before and after treatment. The simplified Fugl-Meyer motor function rating scale was used to evaluate motor function, modified Barthel index was used to evaluate activities of daily living, and MOS 36-item short form health survey was used to evaluate quality of life;and 0-100 digital simulation assessment was used to evaluate patient satisfaction after treatment. Results The simplified Fugl-Meyer motor function rating scale score, modified Barthel index score, MOS 36-item short form health survey score and patient satisfaction rate after treatment in diversification group were significantly better than those in routine group: (76 ± 4) scores vs. (63 ± 3) scores, (65 ± 3) scores vs. (52 ± 4 ) scores, (57 ± 7) scores vs. (44 ± 6) scores, (92 ± 5) scores vs. (77 ± 3) scores, and there were statistical differences (P<0.05). Conclusions Both kinds of rehabilitation model can promote functional recovery in patients with cerebral infarction, but diversification rehabilitation team model is better than conventional model.

<p><b>OBJECTIVE</b>To investigate the effect of oligochitosan in promoting intestinal absorption of protoberberine alkaloids in extracts from Corydalis saxicola total alkaloids.</p><p><b>METHOD</b>The in vitro single-pass intestinal perfusion model in rats was established to study the changes in absorption kinetic parameters of dehydrocavidine, berberine hydrochloride and palmatine chloride in C. saxicola total alkaloids after the addition of different concentrations oligochitosan and evaluate the effect of oligochitosan in promoting intestinal absorption of the drugs.</p><p><b>RESULT</b>The concentration of oligochitosan had different effects on the absorption rate constant (Ka) and apparent permeability coefficient (Peff) of the three active component in rat intestines. Ka and Peff in 0.5% oligochitosan group significantly increased, indicating a stronger effect in promoting the absorption.</p><p><b>CONCLUSION</b>Oligochitosan has a certain effect in promoting the intestinal absorptions of protoberberine alkaloids in C. saxicola total alkaloids.</p>

Objective:To determine the basic characteristics and clinical manifestations of hospitalized patients with psoriasis in Hunan, and to provide reasonable reference for the etiology and treatment of psoriasis. Methods:Totally 190 patients with psoriasis from January 1, 2012 to December 31, 2012 treated in the Department of Dermatology, the Second Affiliated Hospital of Central South University were retrospectively analyzed. The data were analyzed by SPSS17.0. Results:The ratio of male to female inpatient numbers was 3.13:1, aged was between 40 and 70 years. The most common subtype of psoriasis was psoriasis vulgaris (64.73%), followed by psoriatic erythroderma (18.42%). The distribution of the subtype of psoriasis did not differ by gender. Nineteen patients recalled clearly the cause of proriasis, 5 of which were caused by medicine, and 4 by drinking. Totally 96 cases accompanied with other diseases, 24.21%of which accompanied with cardiovascular system disease. Conclusion:There is no season difference in the hospitalization of patients with psoriasis. there are more male than female inpatients. Treatment of psoriasis should consider clinical classification, drug adverse reactions, and individual factors for individual treatment.

<p><b>OBJECTIVE</b>To explore the clinical effects of combined medial and lateral malleolus in treating severe fracture of ankle.</p><p><b>METHODS</b>From May 2007 to January 2011,89 patients with ankle fracture who underment surgical treatment and had complete follow-up, the data were retrospectively analyzed. In this study, there were 66 males and 23 females with an average age of 30.9 years (ranged, 26 to 62 years). Cause of injury included road accidents in 52 cases,sprains from fall down in 17, construction injury in 16 and other injuries in 4. According to type of Denis-Weber, type A were in 13 cases, type B in 24, type C in 52. Operative technique was as follows. Chosen two-incision of combined medial and lateral malleolus, and reduced lateral malleolus fracture firstly, fixed it with 1/3 tubular armor plate or rebuilt-titanium plate. To chose one or two hollow screws according to the size of fracture block. If fracture of fibula was syntripsis,it can be multi-segmental fixed. If fracture line of posterior malleolus was no more than 1/3 articular surface of ankle or the bone displacement was not serious, it can be disregarded. And if the lesion level was over 1/3, or shift position more than 3 mm,it can be fixed with lag screw after reduction. Either front-to-back or back-to-front fixed direction was OK. At the third day after operation, the patients were instructed to exercise with affected limb, weight training after 6 weeks.</p><p><b>RESULTS</b>Eighty-nine patients were followed up from 6 to 24 months with an average of 16 months. Healing time of the fracture was 10 to 12 months. According to standard of the Baird-Jackson scoring system, 68 cases got excellent results, 18 good and 3 fair.</p><p><b>CONCLUSION</b>Surgical treatment is quite effective for severe fracture of ankle. It should be chosen correctly according to types of the fracture and degree of ligament injury. At the same time, damage degree of the ligament around the ankle should be considered and properly treated.</p>

RESUMEN

Objective To investigate the value of posterior short segment peg-stick system plus pediele screw fixation in treatment of thoracolumbar fractures. Methods There were 38 patients invol-ving fractures at Tn in three patients, at T_(12) in 11, at L_1 in 15 and at L_2 in nine. There were three pa-tients with simple fractures of vertebral compression, 35 with burst fractures, 33 with thoracolumbar frac-tures combined with paraplegia. The patients with simple fracture of vertebral compression were treated by posterior short-segment peg-stick system plus pedicle screw fixation. While the patients with paraplegia and intraspinal occupancy were treated by posterior decompression, vertebral enlargement and bone graft-ing (between zygopophysis and transverse process) after fixation. Results All patients received a fol-low-up for average 14.5 months, which showed accurate position of pedicle screw and fine contour of the injured vertebra after reduction, with no loosening or breakage of fixators, chronic lumbar pain or loss of the height of fractured vertebra. Of all, 33 patients were accompanied by paresis. There was one patient at Grade A, six at Grade B, 16 at Grade C and nine at Grade D before operation. But there was one pa-tient at Grade A, one at Grade B, five at Grade C, 10 at Grade D and 16 at Grade E. Conclusions For thoracolumbar fractures especially vertebral burst fractures, posterior short segment speg-stick system plus pedicle screw fixation is an ideal treatment method, for it has the following advantages: no obvious loss of the height of the vertebra after operation, protection of the intact intervertebral disc under the in-jured vertebra and minor effect on range of motion of the lumbar vertebrae.

<p><b>AIM</b>To study the chemical constituents of Caragana intermedia.</p><p><b>METHODS</b>The compounds were separated by chromatography methods, their structures were identified by spectral analysis.</p><p><b>RESULTS</b>Ten compounds were isolated and identified as 5,7,4'-trihydroxy-3,3'-dimethoxyflavone (1), 3,5,7,8,4'-pentahydroxy-3'-methoxyflavone(2), puercetin(3), limocitrin(4), 5,7,3',4'-tetrahydroxy-3-methoxyflavone(5), 7,3',5'-trihydroxyflavanone(6), 5,7,3',4'-tetrahydroxy-3,8-dimethoxyflavone(7), butein(8), liquiritigenin(9) and 5,7,4'-trihydroxy-3,8-dimethoxyflavone(10).</p><p><b>CONCLUSION</b>Compound 6 is a new compound and the others were obtained from this plant for the first time.</p>
